A myomectomy is an operation performed to remove fibroids from the uterus. In myomectomy, the uterus is not removed. The aim of myomectomy is to reduce the symptoms and increase the chance of pregnancy by removing all fibroids in the uterus. Myomectomy can be performed by open surgery (laparotomy) or closed surgery (laparoscopy and hysteroscopy). Which surgical method will be used depends on the size, location, and a number of fibroids.
Laparoscopic myomectomy
Laparoscopic myomectomy is a minimally invasive surgical procedure used to remove subserous and intramural fibroids. Laparoscopic myomectomy is performed under general anesthesia and takes about 2 hours. Four one-centimeter incisions are made in the lower abdomen. The abdominal cavity is then filled with carbon dioxide gas. A thin, lighted telescope called a laparoscope is passed through an incision to see the ovaries, fallopian tubes, and uterus. Long instruments inserted through other incisions are used to remove fibroids. Numerous fibroids up to 15 cm can be removed with the laparoscopic method. At the end of the procedure, skin incisions are sutured. to remove subserous and intramural fibroids. Requires one night of hospitalization and recovery time is around 2 to 3 weeks. Laparoscopic Myomectomy is considered an effective, convenient, and safe procedure to remove fibroids from the uterus.
Some of the benefits of Laparoscopic myomectomy are as follows:
- Smaller Scars
- Lower Blood Loss
- Less Pain
- Short Hospital Stay
- Reduced Risk of Infections
- Faster Recovery
Hysteroscopic myomectomy.
Hysteroscopic myomectomy is a minimally invasive surgical procedure used to remove submucous fibroids located in the uterine cavity. In this procedure, fibroids are removed using an instrument called a hysteroscopic resectoscope, which is inserted into the uterine cavity through the vagina and cervical canal. The procedure does not require any incisions. The procedure is performed under general anesthesia and takes about 30-40 minutes. Hysteroscopic myomectomy is an outpatient surgical procedure. Patients go home after being observed in the recovery room for a few hours. Recovery time is usually only a few days. There are no scars left after the procedure.
Abdominal myomectomy.
Abdominal myomectomy is performed by making an incision of approximately 10 cm in the abdomen. This incision is mostly similar to the skin incision made during a cesarean section. Abdominal myomectomy is usually performed to remove fibroids that are suspected of cancer or larger than 15 cm in size. It is performed under general anesthesia and takes about an hour. Requires 3-4 nights of hospitalization and recovery time is around 4 to 6 weeks.
Pregnancy after myomectomy
Myomectomy can increase the chance of getting pregnant by up to 70% if there are no other infertility factors. Pregnancy can be planned at least 6 months after the surgery. The uterus needs time to fully heal.
